New Patients

Health-Fit Chiropractic & Sports Medicine is a paper-less office and utilizes Medicfusion on-line Electronic Medical Record keeping to give our patients and staff effective, efficient, and time saving procedures to make your experience easy and stress free.

All patients are scheduled for a FREE New Patient Consultation where your condition will be discussed, determined if it is a treatable condition in our office, and details of how your pain or injury will be treated will be given. If you decide to commence with care after insurance and cost is discussed, the rest of your visit will consist of full examination and treatment.

Upon scheduling your New Patient Consultation the staff of Health-Fit Chiropractic & Sports Medicine will send you an email with the login information and PIN # to register as a patient and fill out all pertinent patient information and health history.

Your time is valuable, so we do require all patients to us this system and complete the required information prior to the first visit as it saves 20-30 minutes of paperwork.
Please notify us if you do not have access to a computer and/or email address so we can make alternate arrangements for you.

Auto Accident Patients

If you were in an auto accident, whether or not you have representation, the following forms must be filled out separate from the on-line information. Original copies are required we therefore recommend printing out the forms below and filling the information out prior to your first visit. We understand that the paperwork seems immense, however it will save you 15 minutes and is necessary when working with your auto insurance and/or attorney. We thank you in advance.
